Regional Transition Navigator Services
Assistance for youth and young adults (14-25 years) who are experiencing, have experienced, or are at
risk of experiencing serious emotional disturbance, mental illness, and/or substance use disorders.
Emphasis of services is given to individuals who are experiencing homelessness, who are aging out of
foster care or juvenile detention, or at risk for human trafficking.

Burlington United Methodist Family Services Right From the Start Services
RFTS is a statewide program that helps WV mothers, and their babies lead healthier lives by offering
home visitation services with a Designated Care Coordinator (registered nurse or licensed social
worker). The services are free and support mothers, their new babies and their families by helping
create a safe, nurturing home. Our Designated Care Coordinators discuss any questions and concerns
the clients may have and help them find resources.
