For her vigilance in closing gaps in health disparities
that earned her the title of hospital lead for health
equity; for establishing the “Safety Net Clinic for People
Experiencing Homelessness”; and for her active leadership
role in quality improvement work, including improving
glucose and hypertension control for diabetic patients.
Kathleen Dundee, MSN, RN, CWON
Director of Wound, Ostomy, and Continence Care
Burke Rehabilitation Hospital
Montefiore Health System
For her work to optimize healing and wound prevention
for patients while in the hospital and upon their transition
home, for strengthening clinical partnerships with both
Montefiore hospitals and other institutions in the region
to ensure the quality of care across the patient’s continuum,
and for significantly decreasing hospital-acquired
pressure injuries.

Assistant Director of Nursing
The New Jewish Home, Sarah Neuman (Westchester
Campus)
For creating the Sarah Neuman Nursing School Affiliation
Program, which has grown to train hundreds of nursing
students each year to deliver excellent care to older
adults; and for her exceptional volunteer efforts, including
directing health fairs, screenings, and clothing drives for
communities in need, and implementing wellness
programs in the South Bronx.
