Reflective practice goes digital: Family medicine residents’
use of an asynchronous platform for sharing and engaging
with clinical debriefings

The Department of Family Medicine of Maccabi Healthcare Services in Jerusalem and surrounds
runs an accredited four-year residency program with 37 active residents and 29 trainers. In 2021–22
the Department undertook a novel enrichment project concerning reflective practice via structured
‘debriefing’ of important clinical encounters based on techniques developed in pilot training in the
Israeli Air Force. These written ‘debriefings’ were shared via an asynchronous smartphone-based
digital platform and peers could engage with them through written feedback and further reflection.
To evaluate the engagement and experience of the residents with this sort of digitally shared reflective
practice, data on ‘debriefing’ posts, views and responses was collected and analysed and pre- and
post-project surveys were administered to all participants. Over the three months of the project, 56
debriefings were shared and these were viewed 1962 times, generating 149 written responses. Major
themes that emerged were ‘medical errors’, ‘coping with emergencies’ and ‘patient psychological
complexity’, among others. Reflective practice and sharing of clinical experiences increased among
residents through the life of the project in parallel forums (Balint-like groups and mentor meetings).
Based on this pilot project, it would seem that an asynchronous digital platform for sharing clinical
‘debriefings’ can facilitate residents’ professional development if used consistently.
