Thriving in a crisis: Effective and sustainable
practice models

Traditional models of general practice, regardless of country or heath system type, are struggling
to cope with an ageing population, increased chronic disease and growing societal expectations
for GPs to be more available, accessible and efficient. Amid the global primary care crisis, it is
critical that general practices use sustainable models of care that improve patient outcomes and
clinician satisfaction. A GP-led, patient-centred, locally informed approach that uses all available
multidisciplinary services should form part of the solution.
Instead of waiting for governments to dictate a top-down ‘solution’ to primary care reform, it is
essential to implement innovative and sustainable practice models at the local level. This will enable
us to capitalise on the value of current primary care systems rather than wait for macro-level policy
recommendations that are often well intentioned but have a limited understanding of the importance
of local factors.
This roundtable will engage attendees in a discussion of the key characteristics of responsive and
adaptive GP-led models of care. Some of these key characteristics which fall under the domains
of the Learning Health Systems framework can be harnessed, including optimising the use of the
multidisciplinary primary care team, utilising digital systems and communication technologies to
enhance the care experience and the development of a leadership-inspired culture of learning.
The presenters will provide real-world examples from their practices and local contexts and from their
research program that highlight the challenges and benefits of implementing these GP-led models of
care. Small-group discussion will allow attendees to contribute their ideas and practice experiences
and see how they might align with the framework provided and build on the studies and theories
assembled to date.
