Strengthening family health services in a tertiary hospital in
the Philippines: Changes, collaborations and challenges

Background
An ideal family practice provides continuing and comprehensive medical care with the emphasis on
the family unit. Family physicians are expected to be competent in all health-related matters, serving as
effective communicators, counsellors, collaborators and patient advocates. In the National University
Hospital of the Philippines, with which training, service and research are delicately balanced, the
delivery of psychosocial and family-focused services is often overshadowed by purely biomedical
concerns. This presentation aims to discuss the initiatives and interventions done by the team of
consultant-trainers and resident physicians to improve the delivery of family health services and
strengthen collaborations with other specialities, which are all done within the confines of a busy
primary care/family practice.
Methods
Several initiatives were done by the team of consultant-trainers and resident physicians in order to
improve the delivery of family health services from the year 2022–23. Family health services include
individual and family counselling, wellness, lifestyle interventions, weight management, smoking
cessation and geriatric care, while the collaborative care with other specialities involves the care
of families with special needs and geriatric patients with fractures. Strategies include increased
engagement and dialogue with stakeholders, creation of a technical working group to improve
protocols, facilitation of set weekly schedules for the delivery of family health services and facilitation of
workshops to improve the skills and knowledge base of resident physicians. Active collaborations with
other specialities (developmental paediatrics, orthopaedic surgery, pulmonology) were done utilising
a patient-centred, family-focused and community-oriented approach. Upgrades in health information
systems for data management and reinforcements to the health workforce were also done.
Outcomes
Improvements in service delivery were noted with the implementation of these strategies. Collaborative
efforts with other specialities and interest groups were further solidified. Plans for the eventual delivery
of family health services in the inpatient setting are to be further explored.
