The problem is that EMS is inundated in changing “the culture “and they are doing it reactively than proactively. Unless the commitment is
made to change things, we will basically become a load and go; you call, we haul. Maybe
even a honk and wave where you don’t even
get out of the cab and the patient gets in the
back of the truck. No longer will assessments be performed and from that, a rational decision regarding the level of care a patient should receive

is based on an initial assessment. That is a disservice to the many talented EMT/Paramedics in
our system. If anything, we should give them
every tool and medication and empower them to
use them.
From my own observations and working side by
side with them, I know we have a great system.
I know we have talented personnel who lack collective commitment to make them better at what
they do. With the right commitment of resources, both in training and oversight, we could be
greater. There was a time when we had a partnership with Harvard University (TIMI-19). This
was when the Medical Director advocated for us
to push pre-hospital thrombolytics in the 昀椀eld.
With advancements such as whole
blood and pre-hospital ultrasounds
will we once again become a leader in EMS or just a follower?
