3. LIMITED REVIEW: This is similar to a FULL REVIEW as well, but this does not require
Public Health Council review because these projects are less than $15 million(hospitals)
or $6 million (all other facilities), e.g., minor construction, medical equipment
modifications; add services, etc. Typically, these are the quickest reviews.
4. CONSTRUCTION NOTICE SUBMISSION: This is only used for projects that do not
affect clinical spaces, such as infrastructure or equipment replacement.
Note: Few differences exist between the architectural requirements of the various types
of reviews. It is the same effort for all.
REVIEW SUBMISSIONS:
1. SCHEMATIC SUBMISSION AND DESIGN DEVELOPMENT SUBMISSION:
The SD represents about 30% complete with contract drawings, and the DD is about
60% complete. Requirements for drawings for both submissions are indicated on form
DSG 1.0 and Schedule 6 for the SD submission. We submit Schedules 6, 7, 8, and 10 for
the SD review.
We download the forms from the DOH website to ensure we have the latest ones
(https://www.health.ny.gov/facilities/cons/). The documents did not change for the
longest time, and then they changed frequently for a short period, so constantly referring
to the DOH website is a good practice.
SCHEDULE 6 takes the place of the architectural narrative and specifies what drawings
are required. If a project is complicated, the architect can supplement the information
required in Schedule 6 in an attachment.
The Project Management reviews the information when a project is submitted to ensure
all the required documentation is completed (not for content). If something is missing,
they will not process the CON until they get the missing information.
SCHEDULE 7 is equivalent to the NYS SEQRA and is required on all projects. Generally, if
the project is not large, all items will be negative, which means they do not affect the
project. The one exception is that Schedule 7 requires identifying whether the project is
in a flood zone. If it is, the Elevation Certificate (FEMA Flood Insurance) shall be
submitted with the application.
SCHEDULES 8 & 10 have to do with the cost of the project.
Clients usually have information to add to Schedule 8, so the architect will send the
document in an Excel format to the client so that they can make changes. Schedule 8
