Other Uses of Protected Health Information
Other uses and disclosures of protected health information by the Plan that are not covered by this Notice or the laws that
apply will be made only with your written authorization. For example, in general and subject to specific conditions, the Plan
will not do the following without your written authorization:

Use or disclose your psychiatric notes;
Use or disclose your protected health information for marketing; and
Sell your protected health information.
If you provide an authorization to the Plan to disclose to others, you may revoke it in writing at any time. If you revoke an
authorization, the Plan will no longer use or disclose protected health information about you for the reasons covered by your
written authorization. However, the revocation will not be effective for action that was taken in reliance on your authorization.
