Personal Safety
41% of incidents identified antecedents related
to the Security domain. Of these 5 people were
frustrated by challenges around fluctuating mental
capacity, deprivation of liberty safeguards, and risk
assessments that prevented them from doing things
that they wanted to do.
This has been a theme carried over from 20222023 and is very challenging from an organisation
perspective also. It represents the complexity of
some individuals who have uneven skill bases that
lead to them seeking experiences such as work,
relationships, leisure or social opportunities but are
unable to keep themselves safe, and are not the type
of experiences they want or accept to have support
in. These situations can require complex support
decision making and often require multi-disciplinary
teams to share and manage risk plans.

gives people more independence and privacy, and
prompts support when needed.
