Forms needed for Camps
Important to Know:
o Health & Consent Forms are required for ALL camping activities, see
Camping Health, Consent and Release Forms on Staff Resources.
o There are different reporting forms needed for different types of
camps, and the decision tree on the next page will help you to
select and submit the right forms for the type of camp trip you have
created (in addition to the required Health & Consent forms).
o Links inside the decision tree will take you directly to pages with the
reporting forms you need, so you can follow the way that works
best for taking your kids to camp!
