Purpose of Community Health Needs Assessment
Verdant Health Commission undertook a Community Health Needs Assessment (CHNA) to better
understand how our community defines health, what barriers and opportunities exist in the
community, and identify meaningful solutions that we can support. CHNAs help the Verdant Health
Commission understand the community’s health needs and develop strategies to meet them, in
collaboration with our community partners. The CHNA will be used to achieve the following:
