KIDNEY HEALTH & EDUCATION CENTRE
Funding needed: $8 million
Your loved one’s health is of utmost importance,
especially when they are diagnosed with later-stage
chronic kidney disease. In-hospital care is crucial
during the first month of treatment. This critical
period involves education, personalized support,
and continued discussions to help patients make
informed decisions about managing their chronic
kidney disease.
The Kidney Health and Education Centre will
consolidate kidney health services into one central
location on the second floor of St. Paul’s Hospital.
This consolidation will simplify access to information
and support for patients and their families, while
improving processes for doctors, nurses, and support
staff. Additionally, this will create the space required
to establish the new Transition Program. This
innovative program delivers patient-centered
care and personalized education to patients
managing kidney failure, enabling them to
navigate their healthcare journey confidently.
The Transition Program provides intensive
support and education tailored to the
individual needs of each patient. The
program empowers patients by giving
them the skills and knowledge needed
to manage their kidney treatments
effectively. Patients graduate with
the confidence to continue their

care in their communities or even in their own
homes. This approach reduces patients’ reliance on
hospital care, leading to cost savings and improved
patient outcomes.
The Ministry of Health is in full support of the expansion
of kidney health programs, and the Saskatchewan
Health Authority has forecast to include the increased
cost of operations into future budgets. With your
generous support, we can establish the Kidney Health
and Education Centre and the Transition Program
at St. Paul’s Hospital, providing critical care
and support to patients with
chronic kidney disease.
